junge Erwachsene mit Hilfebedarf, darunter insbesondere Care
Leaver*innen (junge Erwachsene aus Heimerziehung oder
Pflegekinderhilfe), geraten im Landkreis Hildesheim in das
Bermuda-Vieleck des Hilfesystems. Es gibt zahlreiche
Angebote, aber es fehlt an Vernetzung und guten Verfahren
für die Begleitung der Übergänge (Wohnen, Ausbildung etc.). </span><br

Fachtag wird das Hildesheimer Übergangsmodell zur
